The patient-centered essence of the Medical Home practice model is an ideal fit with the mission of quality care and patient safety that drives AAAHC. We offer two options for organizations to achieve recognition as a Medical Home.

NEW! Medical Home certification

As an alternative to full AAAHC accreditation, you can achieve Medical Home On-Site Certification. Like our accreditation program, certification includes an on-site visit by AAAHC surveyors. Transformation of a primary care practice to a patient-centered Medical Home takes time. We believe that we can best support this process at the point of care. After all, how can you assess patient-centeredness except from the point of view of the patient?

The AAAHC Medical Home On-Site Certification Handbook provides specific Standards that focus on:

patient rights and responsibilities
organizational governance and administration
the patient/care team relationship
comprehensiveness, continuity, and accessibility of care
Clinical records and health information
quality of care

Medical Home accreditation

Think of AAAHC Medical Home accreditation as the highest achievement for primary care. It represents the most comprehensive survey available, conducted at your facility by experienced AAAHC surveyors. The process includes a review of your staff, your facility, your equipment, your procedures, and the coordination of your care procedures

AAAHC surveys are unique in that they are peer-based; conducted by professionals who are experienced ambulatory health care providers. During the actual survey, we work with you collaboratively and consultatively to assure that your patients receive high-quality care consistent with AAAHC Standards for the ideal Medical Home model.

AAAHC Medical Home accreditation is a symbol of prestige that you can be truly proud of; a mark of excellence honoring the fact that you adopt best practices and comply with nationally recognized Standards of care. For your patients, it provides comforting reassurance that they will receive high-quality care when they need it.
