AAAHC offers a unique peer-based review process founded on a collaborative, consultative, and educational approach that is ideal for Patient-Centered Medical Home certification.

This program was developed for smaller organizations seeking Patient-Centered Medical Home recognition as an initial step in their quality improvement journey. Standards are based upon the tenets of Medical Home and core requirements for patient safety and quality. The certification survey will focus on the delivery of care as a Patient-Centered Medical Home across all elements of your organization.

Surveys tailored to your organization

AAAHC tailors your Patient-Centered Medical Home certification survey to the type, size, and range of services offered by your organization. AAAHC determines the length of the onsite visit and the number of surveyors based on your Application for Survey and supporting documents. These factors determine your survey fee.

Qualify for certification

AAAHC considers a Patient-Centered Medical Home on an individual basis. An organization qualifies for a certification by meeting specific eligibility criteria.

Earn your certification term

AAAHC awards Patient-Centered Medical Home certification for three years when we concludes that the organization is in substantial compliance with the Standards and when we have no reservations about the organization’s continuing commitment to provide high-quality patient care and services consistent with the Standards. Organizations may receive a three-year term with intracycle activities required for continued assessment of ongoing compliance with the Standards.

Post your public notice of survey

AAAHC reminds all organizations that the policy requires that a Notice of Certification Survey be posted prominently throughout the organization for 30 calendar days prior to the schedule survey date(s), with the exception of random and discretionary surveys.

Understand a denial or revocation of certification

AAAHC denies certification to an organization when it concludes that the organization is not in substantial compliance with AAAHC Standards and/or policies and procedures.

AAAHC reserves the right to revoke or deny the certification of any organization at any time without prior notice. Your AAAHC account manager will help you navigate the requirements to remain in good standing.
