The Medical Home acts as the primary point of care for the patient and the relationship between the patient, his/her family, as appropriate, and the Medical Home sets the foundation.
Accreditation with Medical Home provides an in-depth onsite assessment from both the organizational structure/systems perspective and from the perspective of the patient.
AAAHC surveyors will evaluate your health center’s approach to patient-centered care for all services delivered throughout your organization. This all-in-one survey includes a review of applicable Standards within the accreditation program.
