A Medical Home provides services that are patient-centered, physician, nurse practitioner, or physician assistant-directed (as permitted by state law/regulation), comprehensive, accessible, continuous, and delivered to meet the needs of the individual patients served.

The Medical Home acts as the primary point of care for the patient and the relationship between the patient, his/her family, as appropriate, and the Medical Home sets the foundation.

Accreditation with Medical Home provides an in-depth onsite assessment from both the organizational structure/systems perspective and from the perspective of the patient.

AAAHC surveyors will evaluate your health center’s approach to patient-centered care for all services delivered throughout your organization. This all-in-one survey includes a review of applicable Standards within the accreditation program. An accreditable Medical Home provides accessible, comprehensive, team-based continuity of care and is focused on quality and safety.
