Delivering team-based care in the Medical Home
Whether or not your primary care organization has formally adopted the Medical Home model of care, chances are that you provide (or are beginning to deliver) patient care using a team-based approach. In such an environment, team members play meaningful roles in ensuring quality of care, as well as patient and staff satisfaction. Do your team members understand their roles? How well do they function as a team? This session provides valuable insight into successful implementation of team-based care.
